Procedure

A Drivers (HGV) medical exam is a legal requirement for anybody looking to drive vehicles open to the public. All people wishing to drive vehicles such as a taxi, bus, lorry must have passed an examination with a registered doctor.

At Skin Sense GP Clinic our Doctors can perform a complete range of medical tests for your required drivers medical.You will need to bring a D4 medical form with you to your appointment (which can be downloaded from the DVLA website) as well as photographic identification such as a photo driving license or passport.If you wear glasses or contact lenses, you must also bring your most recent glasses prescription.

In order to complete your medical, you will need to bring a copy of your GP records. Our receptionist will provide you with a letter to give to your GP practice requesting this, if required.

Medical Assessment

Your medical assessment will be performed by our experienced Doctor.
The doctor will ask about any relevant current and previous health issues.

The doctor will perform a physical examination including eyesight test, blood pressure check and a general examination (Drivers who currently need glasses for driving are permitted to have their eyes tested within the last 12 months, You must bring the prescription to your appointment with you.)

The completed medical form will be given to you to take home.
